You may also want to visit our website on inspection findings, which contains additional information on complaints that were noted during routine survey inspections conducted by the Minnesota Department of Health (MDH).

Search Tips

You must enter information in each of the five steps.
If you're unsure of what you are looking for, select the "all" categories.
To find all the investigations for a facility, you must do three searches. One search for each finding type (substantiated, unsubstantiated, and inconclusive).
If you're unsure of the full name, only put in the part of the name you know. If you make a mistake on the name, the system will act like there was no finding.
